Any conditions NOT identified in the HRA are considered resolved or are unable to be confirmed.\nMember is a very poor historian and so her cousins Mary Jane and Kimberly were present and answered the majority of her questions.  The member is not able to read and so she has her medications delivered in a already packaged set but she does not remember when or what to take.  Also, since she is unable to read she does not know what or when to take any of her medications.  Her creams that are supposed to be for her rashes she will put them on her whole body like lotion because she is often does not know how to take them.  She will try to find someone to help her read her medications and paperwork but it is not consistent that she has someone.  The member has a history of falls and seizures and since she lives alone she is not able to tell when her last seizure was or remember if she fell.  The member has a history of poor vision and is not supposed to be driving but still sometimes will drive to her appointments because she has not way to get around to her appointment unless she pays someone and does not have any money.  Her cousin states that the member does not eat unless she comes to her house or she sends her food because she is not able to cook for herself.  She is currently on seizure medications and it was just discovered that she has not been taking the correct dose.  Will send referral because member could really use a skill builder or and aid to come and assist her daily to make sure she is taking her medications and getting to her appointments ontime.  Lastly she is on several medications and some of them do not seem as if they are needed any longer.  Most recently she had a medrol dose pack but did not take any of them because she could not read the titration dose and did not know how or when to take it.  Will send referral.
