The patient states that she is currently in a Tier 2 plan and would like to be moved into a Tier 1.  Also, there are many times when the patient has been out of food and did not have the proper money to afford more food.  The patient has reported that she has had many times where she wishes to go out and is unable to properly ambulate due to her not having a motorized or electric wheelchair.  She would like to inquire about getting assistance with a motorized wheelchair.  In her home due to her difficulty hearing she had a fire alarm system set up the for hearing impaired.  When the fire department came for a neighbors fire the accidently cut her alarm system off and she needs for it to be connected back because otherwise she will not be able to know there is a fire alarm.  She was told that she needed new hearing aids but reports that they along with her CPAP are too expensive for her to afford.
