Member states she was prescribed O2 one year ago at night, because testing indicated lower O2 at night. She stopped wearing the O2 at night 2-3 months ago because the concentrator makes too much noise.  She is able to sleep well.  Her SOB is the same, always worse with activity.  She does not have a pulse oximeter.  She denies a cough or any known hx. of asthma or COPD.  She had an appt. today but  had the HRA today so cancelled and plans to reschedule. She feels stable at this time and feels like she gets support from her long time partner Robert.  She has never been hospitalized for depression and has never been suicidal. She is on Oxycodone 5MG one tablet Q8 hours for the pain.  Member was placed on Calcitonin Salmon NS.  She feels very unsteady on her feet and states she has gotten weaker in her arms and legs over the past year and the rolling walker does not help.  She would like to get a small wheelchair because of the balance issue and her falls. She is being fitted for a back brace.  OthoVA has referred her to Neurology and she is awaiting a call for the appt.  Continues her SOB since 2016 when she had pneumonia txed. as an outpatient with three different antibiotics. She is being followed by Pulmonologist and last appt. 7\/2023.  States PFT's 7 months ago without any particular diagnosis.  She states some test showed low O2 at night and she has been prescribed O2 at night x 1 year, but has not worn it for 2-3 months due to loud O2 concentrator disturbs her sleep.
