Currently he is doing well and does not have a flare up or crisis of sickle cell. Member has had cervical stenosis and had cervical fusion on the same. Since that time he continued with some neuropathy in BUE but not as severe as prior to the surgery. He was being treated in Pain Management in the past and his last office visit was in 1\/2023. He does not care for the pain management center that he was being treated at due to a provider giving him an injection for cervical spine pain and member stated that the provider that provided the injection hit a nerve in his neck and since he has not had any relief from nerve pain or bone pain. Member has had some concerns about not being referred to another pain management center and this has been since 1\/2023. Member had requested to have a Prostate exam and colonscopy done since he had turned 50 years old. He still has not had a prostate exam and has not been informed of any PSA levels. He recently started to have some rectal bleeding and this is the reason that the colonscopy was scheduled for 10\/11\/2023. No referral to Urologist not completed and the member stated that he needs to be referred. Member has to use oxygen at 2 LPM via nasal cannula. He does not have a long enough cord to reach downstairs and he wants to come down steps to be with his family and he would not get SOB. No previous medical conditions to review. Medications reviewed. Referral sent to have the member scheduled to see another Pain Management provider, better communication between PCP office staff and testing center. Member was upset over this and does not know he will continue to see his current PCP. Medicines reviewed with the member. Referral entered to assist the member to obtain a long cord for his oxygen due to limited roaming distance upstairs and not down stairs. He has a 50 foot cord and needs at least 100 feet of cord to reach living room on first floor level.
