She states she has been told especially by the Kidney Dr that the Advil and BC could hurt her kidneys and she should stop taking the two medications. Was in chronic pain management in the past for 2 years, but stopped 2021 when she had a positive urine test, that member states was a mistake which she never took any other drug except the prescribed Loratab which helped her pain.  Has hx. of HA's started at the neck and coming to the back of her head and states she saw a \"neck specialist\" at Ortho VA and no therapy recc. but states was told she had a \"straight neck\".  States she is unsure why she is on the Topiramate, but maybe for her HA's. States over the past year, she has lost 70 pounds +, but does eat irregularly. She checks her blood sugar and uses sliding scale Humalog.  She is unaware of \"previously documented condition\" 11\/2021 \"abnormal findings of diagnostic imaging of other abdominal regions including retroperitoneum\" and unaware of any hx. gallstones and does not have abdominal pain.  States hx. of having a fast heart rhythm x 3 and was seen in ER and given a medication that \"stopped her heart\" and restarted it in regular rhythm.  She was seen by Cardiology and placed on a rate-controlling medication, but in the past year wore a heart monitor x 2 weeks and did not have any heart rhythm problem, and was taken off the rate-controlling medication in 1\/2023 because her BP and HR were too low.  BP now runs 130-140\/70-80 at home.  States no hx. of heart enlargement or CHF by Echocardiogram and does not have any RTC with Cardiology.  Advised member about smoking cessation importance and too high doses of Advil, Tylenol and BC powders and dangers.  Member to keep appt. with Ortho VA and make own Ophthalmology appt.  \n  Will send Urgent Referral for CM to contact PCP about need for earlier PCP appt. regarding high dose of Lasix and earlier lab work (since next appt. 9\/2023).  PCP to also clarify \"previously Documented Conditions\" as noted above and CXR noted above and members chronic pain, skin ulcers, high doses of Advil, Tylenol and BC and risks.
