Problem Conditions","q":[{"a":[]}]},{"t":"Patient Summary","q":[{"a1":"Arrived at the members home and knocked on door. A little girl approximately 2 years old was at the door. Member yelled to come in the house she was not getting up from the couch. She stated that she was trying to get her autistic deaf son down for a nap. A little boy, age unknown, was lying on the couch with the member rubbing and patting his back. Member directed me to sit down in a chair in the front corner of the room so that the little boy could not see me. She stated that if he could see me then he would not go to sleep as he is so fuc... noisy. She stated that she took out his hearing device so that he cannot hear anything so he would go to sleep. She stated that he had been screaming and crying for over a hour and she had to \"make him sleep.\" Little girl was playing with toys in the side of the living room and making a lot of noise per the member (do not feel that the little girl was making too much noise as she was playing with box with toy food in it and was jabbering words and talking to the food). Member suddenly hit the little girl and told her \"to shut the fu... up or she would make her shut up. She said to the little girl that she is in trouble and is suppose to be sitting in her chair and not touching toys. The member proceeded to throw toys onto floor away from the little girl. Toddler became upset crying and the member slapped her on the back very hard and stated that she would give her something to cry about. The little girl sat at the play table crying with her head down then she proceeded to come to me and held her arms up for me to pick her up. I asked the member if I could pick her up and she stated she did not care about her. Member continued to rub and pat the back of the little boy in a continued attempt to get him to sleep. The little girl was sitting in my lap for a while and had her head on my arm holding my arm. Phone rang and the little boy on the couch attempted to sit up when the member was answering her phone. She pushed him down into the couch and stated \"fu... you (pointing to me) are causing him to not sleep and the phone also. Little girl repeated the member and stated fu... several times. Tried to redirect the little girl from saying that with her stuffed animal she was holding. Member answered phone and had the same on speaker phone. A female was on the phone and stated her name that she was calling in reference to a child' grades and wanted to return the members phone call. Member asked the female if the specific child had passed his SOL and the female stated that the child did not and his grades were not high enough to pass to the next grade. Member became angry and stated that the school system did not teach him good enough and was treating him bad. Female on phone stated that the student had not been to school as he should, failed his SOL in math and another subject?? along with being disruptive at school. Female reminded the member that the student had several suspensions that caused also a concern if he was able to pass to the next grade. Female stated that the student would not be promoted and that the decision had been evaluated by the school board members and that they were in agreement. Member started yelling at the female and then stated that she would get the teachers and school system for not teaching the student. The little boy moved and attempted to sit up and the member slammed him back down into the couch. Little girl got down from sitting in my lap and the member started yelling at her to get to the table and sit there until she was off the phone. While sitting patiently in the chair that the  member told me to sit in stated that she needed the information needed about her health. I asked her what exactly she was referring to so that I could answer her questions and then the member told the female on the phone to hold on and then she proceeded to curse, scream and threaten me stating that I did not known what in the he... I was doing and probably could not do my job as I was crippled. Advised the member that at that time I needed for her to have the visit rescheduled to another day and maybe an earlier time due to her son's nap time. The member stated that I could ask her the assessment questions. Proceeded to ask the member the first portion of the IHA. Was going along with the assessment and the member abruptly stopped me and stated that she did not need to answer any of my questions, she wanted to only have the prescription to her incontinent supplies and a narcotic refill. I explained to the member that I would and could not write any prescriptions for her as this was an IHA. The member then continued to scream and curse at me. Member was still on the phone with the female on the phone that was informing the member that she was finished with her conversation and she was hanging up the phone. Member told me to get the fu.. out of her house now and I stated ok thank you so much for her time. Member stated that I wasted her time and she would not want to do the visit again. Got up from the chair that I was told to sit in and walked to the front door. Little girl said to me \"stay stay.\" Said to the little girl that I had to go see other patients. The member stood up and started coming towards me and stated \"get the fu..ing he.. out of her house bit... and then slammed door when I was going down steps at the front entry.
