Member also has COPD, asthma and uses his Albuterol inhaler 1x week.  Has not used Advair Diskus x 7 months.  Continues to smoke, but has quit at times, but starts to smoke again with stress. Is having low back pain x 3 months he thinks from being on equipment with movement.  Hx. of herniated lumbar disk repair x 2 in the 1990's.  Hx. of colon resection in 1990's from diverticulitis and colon polyps.  Has a surgical abdominal hernia that stays sore for a years and states he has had it evaluated for surgical repair and not recommended.  He lost weight down to 240# but has gradually regained to 280# over the past 5 months. Has hx. of seizures, last one occurring 2021 described as jerking all over and confusion x 20 minutes afterward.
