Member has been going through a lot of issues in the recent past. She had to move out of her apartment due to not being able to pay her rent and bills. She has not been able to work as she once was doing to her peripheral neuropathy and unable to walk. She has started using a rollator to ambulate the short distance that she does work. In the past she was taking lots of Tramadol, Vicodin and too much Gabapentin that caused her to have an addiction to the medicines. She stopped the opioids and is only on a small dosing of Gabapentin. She is taking Suboxone currently that is supplied through an outpatient center.  She is tolerating the medicine well. Previous medical conditions reviewed with the member. Medicines reviewed with the member and the member is very knowledgeable of her medicines and the effects of each one of them.  Once the member lost her apartment she moved into her daughters apartment. Her daughters apartment has bedrooms that are only upstairs and this makes it difficult for the member. She can go up and down the stairs but this is a hard task for her to complete.  Member has had some issues with obtaining food in the recent past. She only receives a limit amount of Food Stamps and this helps but it is not enough. Reports that when she walks she does not feel her feet due to numbness and she falls often. She states that at times she will walk out of her shoes and she does not even realize the same. Member needs to locate a new PCP as her current one is leaving and the location is not close to her and her limited transportation. While completing the depression screen during the assessment it was found that the member had severe depression through the score of 29 on the screening tool. Member states that she feels that with the struggles that she has had in the recent past and her declining health, she has given up and has fallen into a deep depression. Member requested for assistance in locating a psychiatrist for her to be seen by so that she can overcome this and make her health better. Referral will be completed for the member to request a urgent mental health evaluation to assist with severe depression, food disparity, high fall risk and possibility of having a Life Alert. Also assistance in locating a new PCP for continuity of care.
