He recently received a left AKA  due to Gangrene infection.  The member has a wheelchair and has been having difficulties because there is no ramp or lift in his home.  He reports that if his older son is not available to help him go up and down the stairs he has to sit down and scoot on his bottom to get up and down the stairs.  The member starts PT soon and is hoping this will help him to be able to ambulate safely.  In the past year the member has had several falls and worries about falling if not in his chair.  The member does not have job right now and reports that he has tried several times to get disability but has not been successful.  Currently the member reports he is having food disparities.  Discussed with member this his PHQ9 score is elevated and shows that granted he may not have severe depression his depression is not well controlled.  Discuss with member that since he currently is taking Buspirone for his smoking cessation it may benefit him increase the Buspirone so that it helps with his depression as well.   Member has been a widow for 6 years, and lost is wife to cancer.  Advised member that he should also get a counselor because it will help him cope with his depression as well.  Will send referral.
