Verbal consent was received by the patient to conduct this virtual visit. Patient identity was verified by address and DOB. Palpation, percussion and auscultation portions of the assessment as well as the Monofilament Test were unable to be performed due to the virtual performance of this assessment. Due to the virtual manner in which this assessment was conducted the vital signs were either self reported by the member, obtained by the member or unable to be completely obtained. The member was provided counseling for Preventive Health Maintenance recommendations. Any conditions NOT identified in the HRA are considered resolved or are unable to be confirmed.
