Verbal consent was received by the patient to conduct this virtual visit. Patient identity was verified by  address, and DOB. Palpation, percussion, and auscultation portions of the assessment as well as the Monofilament Test were unable to be performed due to the virtual performance of this assessment.  Due to this being a telehealth assessment the vital signs were either self-reported by the member, obtained by the member, or unable to be completely obtained.  The member was provided counseling for Preventive Health maintenance recommendations.  Any conditions NOT identified in the HRA are considered resolved or are unable to be confirmed.  The member has been advised to always follow up with their PCP\/specialist with all their health care needs.
